Faculty and Staff Software Request Form

The Faculty/Staff Software Request Form must be signed by all employees if they wish to purchase software to be used on a University-owned computer.

Employees that wish to purchase software for use on a home computer must complete the Microsoft Campus Agreement Work at Home Acceptance Form for UC Faculty and Staff.
