Faculty Email Template for Students

Hello Professor,

I am a student in your (ENTER COURSE HERE). I am registered with Accessibility Resources and I am requesting to use my approved academic accommodations in your course. Attached you will find my Accommodation Form, outlining my approved academic accommodations for the current semester. 

Faculty role in this process 

  • Reply to this email to acknowledge that you received and read my accommodation form.
  • Reach out to me or Accessibility Resources if you have questions regarding the listed accommodations. 
  • Respond in a timely manner to ensure requested accommodations are provided

Student responsibilities in this process 

  • Request accommodations for the class by providing professors with a copy of their Accommodation Form
  • Request on-going accommodations, such as testing accommodations, in a timely manner as needed throughout the semester
  • Discuss accommodations with my professors they request it, with support from Accessibility Resources if needed
