Environmental Health & Safety Issues New Accident Form

The University of Cincinnati has revised its Initial Report on Work-Related Injury or Illness, also known as the University’s “accident report,” in accordance with the State of Ohio’s revision of Ohio Administrative Code 4167-06, Recording and Reporting Injury and Illness.  These changes are effective January 2005.

Environmental Health and Safety asks UC offices to discard all older versions of the Initial Report on Work-Related Injury or Illness, form A-1352(a). 

"Only the 01/05 revision is current," said Environmental Health and Safety Director Jan-Arthur Utrecht. " The Initial Report should be supplied to injured employees through the supervisor or the Departmental Business Office."

Copies of the revised report form are available from Environmental Health and Safety’s Web site at http://www.ehs.uc.edu, and through Central Stores (stock number 30-2400).

"The injured employee and the employee’s supervisor are to complete this report no later than 24 hours after an incident occurs, regardless of the extent of injury or illness," Utrecht said.

A brief on-line training presentation of the University’s Work-Related Injury and Illness Policy is also available through Environmental Health and Safety’s Web site.  All managers and supervisors are encouraged to view this training, which discusses how to help injured employees:

  1. Receive prompt medical treatment of work-related injuries and illnesses;
  2. Report incidents in a timely manner;
  3. Report time off from work and return to work safely and promptly;
  4. File Workers’ Compensation claims;
  5. Assist in identifying and initiating the corrective measures that will prevent further occurrences.

"Please share this information with all employees, and emphasize the supervisor’s role in effective implementation of the University’s Work-Related Injury and Illness Policy," Utrecht said.
